It is Parent's / Guardian's responsibility to ensure that a current permssion form is on file with Journey Church. Please resubmit this form to maintain current information if original contact information changes.
Should you wish to use this electronic permission form, its submission will be accepted as your consent and execution of your permission for the activities of the minor(s) as indicated.
RELEASE FROM LIABILITY/COVENANT NOT TO SUE
In consideration of the aforenamed Minor's participation in the Journey United Methodist Church Childrens and/or Youth Group activities for the calendar year starting January 1, 2025 and ending December 31, 2025, and for other good and valuable consideration, I on my own behalf and as an agent for others standing in loco parentis and on behalf of the aforementioined Minor, make the following covenants and releases:
Journey UMC and its officer, partners, members, agents, servants, and employees, when acting on behalf of Journey, are hereby forever released, acquitted and discharged from any claims, suits, losses, or liabilities whatsoever by reason of injury, illness, or damage to person or property during the course of participation in this activity, including transportation to and from activities of Journey, and in that regard, I hereby covenant on my own and the aforementioned minors behalf not to file, assert or allege any claim or bring any suit whatsoever relative to any such injury or damage.
I have read and fully understand the provisions of the above release and have explained them to said minor. By my signature below or by the electronic submission of this document, I hereby certify and agree that I and said Minor will be bound thereby and I shall defend and hold harmless Journey UMC from any liability thereof by or on behalf of said minor.
PARENTAL CONSENT TO MEDICAL TREATMENT
I, as parent or guardian of the minor(s) herein named, by electronic submission of this document, hereby set forth, that:
in the event of illness or accident occurring to him/her during attendance at all functions sponsored by or attended with Journey United Methodist Church for the calendar year January 1, 2025, thru December 31, 2025, do hereby consent in advance to whatever emergency medical treatment, x-ray, examination, anesthesia, medical, surgical or diagnostic procedure, or treatment is considered necessary in the best judgment of the attending physician or emergency medical personnel. I understand that, in the event of a serious illness or injury which is not an emergency, reasonable efforts to reach me will be attempted. There shall be no cost incurred by Journey UMC or its agents.
PARENTAL CONSENT FORM PHOTOGRAPHY, MEDIA AND WEBSITES
Photography - During the year, there are occasions when photographs of the aformentioned minor(s) may be taken by a volunteer associated with Journey United Methodist Church. Your authorization to use a photo or photos of the aforementioned minor in Journey United Methodist Church publications (brochures, newsletters, etc.) is requested. Names may appear in these publications.
Media Coverage - From time to time the media may cover our events. Whether or not you have concerns about the aforementioned minor participating in media coverage (being photographed, videotaped, interviewed) it is important that we know your wishes. Media coverage by local newspapers may include the aforementioned minor's name (if so, the reporter typically will obtain your consent.)
Websites - Journey United Methodist Church website (www.journeycolumbia.org), Facebook and YouTube pages contains information on our program. From time to time Journey UMC may utilize other social media or print. These tools are used to help the community and participants learn more about our programs and activities. Anyone with Internet access is able to view these pages. Participant photos may be posted on our website to showcase events and activities. If included, identification would be done by first name and first initial of last name only. Photos of large groups, or action photos where participants names cannot be identified, may be posted without parental permission.
